Give dynamic URL and to Fetch custom field value in CTA so it will be very helpful to redirect user on another number with the custom field value.

You can provide custom field variable in url field as parameter of url or as like you want, Custom field will be replaced the value of that subscriber. However will provide instruction there about using custom field also
